Creatine: What the Science Actually Says About Benefits, Safety & Dosing

Creatine is the most-researched, most-effective, and most-misunderstood supplement in all of sports nutrition. As a Certified Nutritionist in San Diego, I get asked about it constantly — so here is what more than 20 years of peer-reviewed research actually shows, without the hype or the myths.

What creatine is and how it works

Creatine is a compound your body already makes and stores in muscle as phosphocreatine. During short, intense efforts — a heavy set, a sprint, a jump — phosphocreatine rapidly regenerates ATP, your muscle’s immediate energy currency. Your body produces about a gram a day and you get more from red meat and fish, but supplementing tops off (“saturates”) your muscle stores so you have more on tap when it counts.

The proven benefits

The International Society of Sports Nutrition’s official position stand calls creatine monohydrate “the most effective ergogenic nutritional supplement currently available” for increasing high-intensity exercise capacity and lean body mass during training (Kreider et al., 2017). In plain English: more strength, more power, better sprint performance, and better training adaptations — in younger adults, older adults, and everyone in between. Creatine and your brain

This is the exciting frontier. Because your brain also runs on ATP, researchers have studied creatine for cognition — with the best signals for memory, attention, and processing speed, especially in people who are sleep-deprived, under stress, or older (EFSA, 2024). I’ll be honest: the muscle benefits are rock-solid, while the brain benefits are promising and dose-dependent rather than settled. But it’s a real and growing area of evidence.

Is creatine safe? (The kidney myth)

This is the big one. After more than two decades and hundreds of studies, there is no evidence that creatine damages the kidneys in healthy people (Antonio et al., 2021). The myth comes from creatine slightly raising creatinine — a marker labs use to estimate kidney function — which is a harmless byproduct of creatine metabolism, not a sign of damage. The most common real side effect is a small amount of water weight when loading, held inside the muscle. One caveat: if you have existing kidney disease or take medications affecting kidney function, talk to your physician first.

How to take it

Simpler than the supplement industry wants you to believe: 3–5 grams per day, every day. Timing doesn’t matter much — consistency does. A “loading phase” (about 20 g/day split into 4 doses for 5–7 days) saturates your muscles faster but is completely optional; you reach the same place in a few weeks at 3–5 g/day.
